How to Apply

Medical Student Application Requirements:
  • A student’s medical school must be LCME or AOA accredited. For international medical schools, the schools must be listed on the “Indiana’s Approved Medical School” list as found on the Indiana Professional Licensing website. Students of schools listed on the “Indiana’s Disapproved Medical Schools” list will not be approved under any circumstances. Any other schools will be considered on a case by case basis.
  • Medical student must be a US citizen or a permanent resident
  • Must be actively enrolled as a 3rd or 4th year student of an approved medical school
  • Students must pass Step 1 USMLE/COMLEX prior to acceptance, others will be reviewed on a case by case basis
  • Rotations must be requested at least 60 days in advance
  • Professional Liability obtained from school
 Student Request Process:
Please note there are separate applications for core and elective rotations. Make sure you use the correct application, complete all sections needed and submit to student.rotations@deaconess.com.

  1. Completed Medical Student Application
  • After the application has been received and reviewed by the Academic Coordinator, you will be contacted with approval or denial via email. 
  • Deaconess utilizes ACEMAPP to onboard and certify students. Once the student has been approved for a clinical rotation, the Academic Coordinator will register the student on ACEMAPP. There is a $50/ annual fee for students. Students must complete online orientation and submit required documentation through ACEMAPP before they can begin their rotation at Deaconess. 
  • Medical Students will be required to submit/complete the following in ACEMAPP:
  1. Malpractice Medical Insurance
  2. Letter of Good Standing
  3. Confidentiality Form
  4. Handbook Acknowledgement Form
  5. Insurance Certification/ Liability Waiver
  6. Immunization Records (TB, Rubella, Rubeolla, Varicella, HepB, Tdap)
  7. Flu Shot (For student completing rotations October-March)
  8. Health Inventory
  9. Student Photo
  10. Background Check
  11. Drug Screen
  12. BLS
  13. Complete Bloodborne Pathogen Assessment
  14. Complete HIPAA Assessment
  15. Complete OSHA Assessment
