Wellness Screenings

Current screening year:  September 1, 2018 - April 1, 2019

Employee Wellness has been working hard on revising our new wellness program for this year. Highlights for this year include a preventative care HRA credit and a new program series category. Please review the Notice Regarding Wellness Program and Notice Regarding Wellness Program Spouse Authorization.


Wellness Screening Information

Wellness screenings will be year-round until the August 1, 2019 deadline. It is recommended to screen prior to April 1, 2018 to allow for time to complete MyWellness Portal activities and earn HRA dollars. Appointment dates and times will continually be added throughout the year.
 
Screenings will largely take place at the Main Campus Wellness Center and our new wellness center at the Deaconess Corporate Center located at 7100 Eagle Crest Blvd. (Deaconess MyChart Building). There will be some appointments also scheduled at Gateway.
 

The Wellness Incentive

An incentive/dollars added back to your paycheck to offset healthcare cost. Both employees and spouses are eligible.  $33.00 for employee/$11.00 for spouse 

Incentive Requirements

Earn your wellness incentive by completing ONE of the following: 

  • Complete bio-metric screening with Employee Wellness 
  • Attend 2 Wellness Coaching one-on-one session
  • Complete Nicotine Cessation Series Program

How to complete the above requirements

  • Best time to screen - October through April 

  • Deadline:  August 1st, 2019 (includes wellness screening, Personal Health Survey & all wellness plugins for HRA dollars)

  • If you are benefits eligible after April 2nd you will be grandfathered in and receive the wellness incentive for you and your spouse (if elected on the insurance).  All employees can earn HRA dollars regardless of hire date. 

Screenings will largely take place at the Main Campus Wellness Center and our new wellness center at Deaconess Corporate Center on the eastside. There will be some appointments also scheduled at Gateway.
 

Earn Reward Dollars

Both you and your spouse will each have the opportunity to earn up to $400 reward dollars for your HRA account. Be sure you pick-up a rewards activity handout at your screening as some activity categories have changed from the previous year.

Employees and spouses can earn up to $400 into their Health Reimbursement Account (HRA) by participating in additional wellness activities.  (Nutrition Counseling, Personal Training, and Wellness Clinics.) 

How to prepare for your wellness screening

Employees/spouses who take Deaconess medical insurance and who are interested in receiving the incentive must complete a wellness screening.

  • A 12 hour fast is required prior to screening appointment for all beneficiaries, unless pregnant at time of screening.
  • Labs can be drawn prior to screening date, please provide printed copy or inform wellness staff that they are available in Epic.   
  • Wellness screening consist of: Height/weight & BMI measurements, blood pressure reading & lipid profile (via fingerstick)
  • Mass Screenings: Held September – December
  • Recommended screen by date: April 1st, 2019
  • You are allowed to take prescribed medications if they are not required to be taken with food.

How to Sign up for your Wellness Screening

Before you can sign-up for a Wellness Screening, you must first complete your Personal Health Survey and create an account. Use the tip sheet “Logging In and Completing Your Personal Health Survey” to get started. Once your account has been completed, you may use the “Scheduling a Screening or Event” tip sheet to sign-up for a screening.
