Employees can sponsor up to two additional members:

  • $45/3 months

  • $180/year 
  • Payroll Deduction ($15/month) - only available for employees with authorized hours
Personal Training
Are you lost when it comes to working out in a gym? Our Deaconess Certified Personal Trainers are here to help! Sessions at Midtown Gym will be offered Monday-Friday between 7:00am and 4:00pm. Client must be cleared by their doctor for physical activity. Pricing and Packages are below. Complete this form to tell us you are interested, and a Deaconess Certified Personal Trainer will be reaching out to the email provided.

Standard Single Session 30min. Hume Body Pod included once per month.
  • $28 (Community, no gym membership included)
  • $20 (Deaconess Employee pricing, must have active gym orientation)
Premium 8- 30min session Package. Hume Body Pod included once per month along with customized coaching and workouts with the Trainerize application:
  • $224 (Community, no gym membership included)
  • $160 (Deaconess Employee pricing, must have active gym orientation)
 

Outside Activity Requirements

Can spouses receive a free gym membership, etc.?
Yes, all beneficiaries who are participating in the wellness incentive will receive a free gym membership.

Liability Waiver

The undersigned herby agrees to waive and hold harmless Deaconess Hospital, Inc., its officers, directors, employees, agents, and independent contractors, ("Deaconess Affiliates") from any and all liability, including but not limited to bodily injury, death, and con- sequential losses or damages sustained on or about the Deaconess Fitness Center premises or sustained in participation of its activities, regardless as to whether such injury or loss was due to my own participation or actions of any Deaconess Affiliate, or any third party. I understand as my participation is voluntary and undertaken on my personal time, and that any injury would not be covered by the Hospital's Worker's Compensation or general liability insurance, but rather would be my responsibility under the provisions of my personal medical insurance program.

I have read and understand this form and the program it describes, and I voluntarily request to participate in the Deaconess Hospital, Inc. Fitness Center Program.


Modified Physical Activity Readiness Questionnaire (PAR-Q)

Regular exercise is associated with many health benefits, yet any change of activity may increase the risk of injury. Completion of this questionnaire is the first step when planning to increase the amount of physical activity in your life. Please read each question carefully and answer every question honestly:

Has a physician ever said you have a heart condition?:

 
When you do physical activity, do you feel pain or discomfort in your chest?:

 
Have you ever participated in a cardiac rehab program?:

 
When you were not doing physical activity, have you had chest pain or discomfort in the past month?:

If you answered yes to any of the above questions, you are electing to utilize the Deaconess Fitness Center without clearance from your doctor.

If your health changes so that you answer yes to any of the above questions, you need to promptly inform the Fitness Center and talk to your doctor to continue your exercise program.
