Community Health Needs Assessment
As part of its commitment to delivering patient-centered care and improving the health and well-being of communities across southwestern Indiana, western Kentucky and southeastern Illinois, Deaconess Health System conducts a Community Health Needs Assessment (CHNA) for each hospital in accordance with the Patient Protection and Affordable Care Act. These assessments help identify each community's most pressing health needs and existing strengths, enabling Deaconess to prioritize initiatives, guide the development of community health improvement plans and support local organizations in their planning and outreach efforts.
Throughout the assessment process, Deaconess actively engages community members and collaborates with healthcare providers, community organizations and local leaders to better understand the region's most significant health needs and priorities. The findings in each CHNA are informed by a comprehensive analysis of more than 200 national and state health indicators, as well as input from community leaders, organizations serving diverse populations and individuals with firsthand knowledge of local health challenges. These insights help guide Deaconess' community benefit investments and inform strategies to improve the health of the communities it serves.
Each Community Health Needs Assessment culminates in the development of a Strategic Implementation Plan (SIP) for the respective hospital. These plans outline the strategies and actions each hospital will take to address the most significant health needs identified through its assessment while aligning with the broader priorities of Deaconess Health System. Together, the plans serve as roadmaps for the next three years, guiding community benefit initiatives, supporting the health of the communities served and advancing the mission of Deaconess.
