Participating post-acute care facilities work closely with Deaconess to ensure consistent and seamless clinical care. (For example, participating post-acute care facilities report their performance to Deaconess and Medicare/ Medicaid on a regular basis.) Our skilled nursing partners have nurse practitioners who work directly within their facilities and track quality metrics to help prevent re-hospitalization, help patients be successful at transitioning home after a skilled nursing stay, and help improve patients’ health and well-being.
Please note that all of the participating postacute care facilities are independent from Deaconess. Also, please be aware you have the right to select any post-acute care facility at your discretion, notwithstanding the joint quality initiative Deaconess has with participating post-acute care facilities.
Medicare Patient Freedom Of Choice
If you are a Medicare beneficiary, you have the absolute right to receive health care services from any health care provider that participates in Medicare, furnishes the services you require, and undertakes to provide these services to you. Your choice of health care provider may, or may not, result in additional out-of-pocket expenses for you. You should consult with your physician and also consider the financial consequences before choosing a provider.
