Care Coordinator Services

You will be assigned a care coordinator to help you with a wide range of non-medical issues that may be affecting your overall well-being. You will meet with your care coordinator within your first few visits.

Consistent Caregivers

Unlike an urgent care, you’ll usually see the same physician at each visit. That way, you can develop a relationship with your doctor based on your specific needs.

Customized Care Plans

You and your team will design a customized care plan that you understand. That includes diagnosis, treatment, education and wellness issues.

Longer Office Visits

We schedule more time for each patient so you don’t feel rushed. Our doctors and staff take the time to listen, so you have time to ask questions and express concerns.

Nursing Home Visits

Our physicians and nurse practitioners provide a nursing home triage service for all of our patients who are in various nursing homes throughout the community.

Senior-Friendly Design

We offer exam rooms that accommodate family members and wheelchairs, corridors with handrails, carpeting throughout the facility for greater traction and convenient parking.

Senior-Sensitive Staff

Our primary care doctors, nurse practitioners, and social workers are focused specifically on the needs of individuals age 65 and older.
