Remote Patient Monitoring

If you've been diagnosed with CHF, you might qualify for Remote Patient Monitoring (RPM). 

What is Remote Patient Monitoring (RPM)?

• A program that allows you to be monitored by a nurse using your personal health kit. 
• You will answer questions about how you feel each day.
• Your information is shared securely through the cellular network.
• The RN is alerted to any issues and contacts you and your provider if needed.

Please note that you must have a physician referral.
