How to Apply - Advanced Practice Provider Programs

Before applying, please ensure you meet the Application Requirements

To request a clinical rotation at Deaconess:

1. Complete the Appropriate Regional Application

Separate applications are available for each Deaconess region. Applicants must complete the application corresponding to the location where the rotation is requested:

• Indiana Region APP Program Application
• Illinois Region APP Program Application
• Kentucky Region APP Program Application

2. Prepare a Letter of Introduction

Your letter should include:

• Your licensure goal (FNP, AGACNP, PMHNP, PA-C, etc.)
• A summary of previous healthcare experience
• Why you have chosen Deaconess for your clinical rotation
• Your anticipated area of practice following graduation

3. Submit Your Application

Email your completed application and letter of introduction to:

Student.Rotations@Deaconess.com

4. Application Review

Applications are reviewed based on educational fit, preceptor availability, and clinical capacity. Applicants will be notified by email regarding the approval or denial of their request. 

Important: A new application must be submitted for each academic semester.
