Credentialing and Privileging
- Copy of College Transcripts
- Verification of Nursing or PA license
- Documentation of national certification
- Current BLS: need to have 90 days from date of employment
- Copy of DEA registration
- Verification of prescriptive authority
- Delineation form requesting credentialing and any special privileging
- 24 months of procedure/activity logs
- Deaconess primary source verifiers will run a background check
- Three references: one from a supervisor, one from a physician, two with same title as applicant
