The Diabetes Care Program is a diabetes education and monitoring program designed to help Deaconess Health Plan beneficiaries better manage their diabetes.
People with diabetes who control their blood sugar have been shown to have better outcomes than people who don’t control their blood sugar.
The Diabetes Care Program is a way for you to learn techniques to control your diabetes by taking advantage of some great diabetes education resources through Deaconess Health System.
MTM Clinic
As part of your Employee Wellness Plan, you may be referred to the Deaconess Pharmacy Medication Therapy Management Clinic. You will have a one-on-one visit with an experienced pharmacist who will review all your medicines, including vitamins, herbal supplements, and over-the-counter medicines, to ensure that they are safe and effective. The goal is to help you get the most from your medicines, so the Deaconess pharmacist has special permission from your doctor to adjust your medicines to make them work the best for you. The pharmacist can also provide education on disease states and explain lab results.
The Deaconess Diabetes Center provides a dietitian and a nurse who serve as educational resources for individuals regarding diabetes management, training, and prevention as well as reduction of complications from diabetes. Classes are held in groups of two or more, but private classes may be scheduled if appropriate. Assessments of all individuals must occur before the initial class. Classes are available at Deaconess Clinic Downtown and Deaconess Gateway Hospital at various times.
Care Program Process Flow

Obtaining Free Supplies:
When you complete your Diabetes required program, you will receive the following Freestyle® diabetes supplies free of charge from the Deaconess Family Pharmacy:
- Glucose meter
- Glucose meter test strips
- Lancets
- Glucose meter control solutions
- Insulin syringes
- Insulin pen needles
Primary Care Physicians should send prescriptions to the Deaconess Family Pharmacy to be filled. Each time supplies are low, contact the Family Pharmacy for a refill. There are no auto-refills for free supplies.
Free supplies are available per 6 months. At the end of 6 months, you must have an A1C recheck. Submit the results to Employee Wellness to extend free supplies for another 6 months.
**Important: If you are currently receiving free supplies, please schedule your wellness screening in October or November to continue the free supplies program. Free supplies will expire November 30, 2021 if wellness screening is not complete. Additional MTM or Diabetes visits may be required to continue free supplies.
