Request a Wound Care Appointment

Deaconess Wound Services provides treatment for chronic, non-healing wounds caused by diabetes, vascular disease, injury, infection or other problems.

If you are experiencing any non-healing wounds, you can request a new patient appointment online with one of our board certified providers. Please fill out the form below to request an appointment.

Appointment Guidelines 

• New patient appointments for patients age 18 or older
• If this is an urgent medical issue, please call our office at 812-450-7700
• If this is an emergency, please go to the nearest emergency room
• A referral from your provider will be required for our services; we will help with any necessary referrals once you request your appointment below.
• If you are already a Deaconess Wound Services patient and need a follow up appointment, please log into MyChart to schedule.
• All appointment requests will be reviewed and subject to change by the wound services referral team.
