    Community Invited to Participate in Health Needs Assessment

    Deaconess Memorial Medical Center is inviting community members to share their voices by participating in our Community Health Needs Assessment (CHNA) survey. The survey is open now through July 26th and will be used to evaluate the overall health needs of the area.

    The Community Health Needs Assessment provides community benefit through shaping future programs, services, and healthcare priorities and improving the health and well-being of communities served by Deaconess Memorial Medical Center.

    We encourage all community members to participate.  Responses are anonymous and take about 10 minutes to complete. To participate, please visit any of the links below.
