Insureds: Employees, Spouses, Board Members and Retirees
This screening offers you an opportunity to know your health status, improve your lifestyle, and stay healthy.
Monday, April 27, 2026 5:30 a.m. – 9:00 a.m. Jasper Elementary
Tuesday, April 28, 2026 5:30 a.m. – 9:00 a.m. Jasper Elementary
Wednesday, April 29, 2026 5:30 a.m. – 8:30 a.m. Jasper High School
Thursday, April 30, 2026 5:30 a.m. – 8:30 a.m. Ireland Elementary
Friday, May 1, 2026 5:30 a.m. – 8:30 a.m. Jasper Middle School
Screening includes:
- Blood Pressure
- Height and Weight
- BMI (Body Mass Index)
- Waist Circumference
- Tobacco Status
- Blood Glucose (blood draw)
- Blood Cholesterol (Total Cholesterol, HDL, LDL, Triglycerides, Cholesterol Ratio) (blood draw)
- PSA (blood draw), optional for men 50 and older or those with a risk for prostate cancer
- Colon Cancer detection take-home kits, optional for those over 50
- Chemistry panel (blood draw)
Optional Add on Blood Tests: Self-Pay
- TSH (Thyroid Stimulating Hormone) (blood draw)
- CBC (Complete Blood Count) (blood draw)
- A1C (Average Blood sugar Levels = 3 months) (blood draw)
COST:
- Basic Screening: Free for all Insureds
- PSA blood test: $22 (optional for men); Free for all Insureds over 50 years old; others $22
- Colon Cancer detection kits: $9 (optional); Free for all Insureds over 50 years old; others $9
- TSH (Thyroid Stimulating Hormone) (blood draw): $14 (optional)
- CBC (Complete Blood Count) (blood draw): $14 (optional)
- A1C (Average Blood sugar Levels = 3 months) (blood draw): $20 (optional)
Appointments will be made every 10 minutes. Twelve (12) hours fast is needed for bloodwork.
You may drink water and take prescribed medications. Blood is take by venipuncture.
