Application Process

Students requesting a clinical rotation at Deaconess must complete the requirements as outlined below and email the documents to Student.Rotations@deaconess.com.

Due to the large amount of clinical placement requests for students and the limited number of available spaces, Deaconess employees and students from local schools receive prioritization.

Nurse Practitioner & Physician Assistant Student Application Requirements:
  • PA/NP student must be a US citizen or a permanent resident
  • Must be actively enrolled in a PA or NP program
  • Rotations must be requested in appropriate timeframe
  • Professional Liability obtained from school
Student Request Process:
   
  • Completed application
  • Current CV or resume
  • A 1-page Letter of Introduction describing the following information:
    • Licensure Goal (FNP, ACNP, PMHNP, PA-C, etc.)
    • Description of previous health care experience
    • Why Deaconess was chosen for student rotations
    • Why you want to be an NP or PA and in what area you want to practice after graduation
    • Name/contact info for clinical advisor
  • Copy of RN license (NP students only)

*Students MUST submit a new application for each semester. No need to resubmit supporting documents*
 

Semester: Application Deadline:
Fall (Aug - Dec) March 31
Spring (Jan-April) August 31
Summer (May-July) January 31

 

  • After the application and supporting documents have been received and reviewed by the Academic Coordinator, you will be contacted with an approval or denial via email.
Before Your Rotation Begins:
  • Deaconess utilizes the online ACEMAPP system to onboard and certify students. Once the student has been approved for a clinical rotation, the student’s school Clinical Advisor will need to create an ACEMAPP profile for the student. Students must complete online orientation and submit required documentation through ACEMAPP before they can begin their rotation at Deaconess. There is a $50 annual fee.
  • NP/PA Students will be required to submit/complete the following in ACEMAPP:
  1. ACEMAPP Profile Photo (this will be used for your badge)
  2. Background Check
  3. BLS
  4. Complete Bloodborne Pathogen Assessment
  5. Complete HIPAA Assessment
  6. Complete OSHA Assessment
  7. Deaconess Confidentiality Form
  8. Deaconess Handbook & Acknowledgment
  9. Deaconess Health Inventory
  10. Deaconess Insurance Certification/Waiver
  11. Drug Screen
  12. Immunization Records (TB, MMR, Varicella, Hepatitis B, TDAP)
  13. Influenza Vaccination (for rotations October-March, annually)
  14. Malpractice Insurance
  15. Photo ID (Driver’s License)
  16. RN License (NP Students Only)
