Deaconess Care Integration ACO Program: Quality and Performance Monitoring Program and Related Initiatives

DCI has established the Quality/Performance Monitoring Program, a program designed to (a) evaluate the performance of DCI and DCI’s ACO network providers in order to facilitate the collaboration of DCI’s ACO network providers in connection with their participation in DCI’s provider network and in DCI’s affiliated ACOs (e.g., OneCare, LLC, etc.); (b) facilitate the quality and improvement activities undertaken by DCI, its ACO network providers and its affiliated ACOs; (c) develop, evaluate and enhance DCI’s population based activities to improve the health of patients, including DCI’s aligned Medicare beneficiaries; (d) develop and enhance DCI’s platform which is designed to enhance the quality and efficiency of the services provided by DCI’s ACO network providers and improve health care in the communities served by DCI, its ACO network providers and its affiliated ACOs; and (e) assist DCI in carrying out its obligations and duties under the Next Generation ACO Model, all consistent with purposes of DCI and in furtherance of the goals of the Next Generation ACO Model. 

In connection with DCI’s Quality/Performance Monitoring Program, DCI implemented initiatives in July 2016, involving DCI, participating DCI network providers, including Deaconess Hospital and Deaconess Clinic, and DCI’s affiliated ACOs.  These initiatives involve the use of data and reports, including Crimson data and reports generated through The Advisory Board Company, for reporting to DCI’s Board of Managers for purposes of furthering the goals of DCI, including the goals of the Next Generation ACO Model (e.g., trends and recommendations with respect to coordinating care of patients, including DCI’s aligned Medicare beneficiaries, for the best possible outcome and efficient service delivery, etc.).
