Deaconess Hospital, working in coordination with DCI, has developed the Digital Care Integration Program, an ACO program designed to achieve the following goals: (a) facilitate the collaboration of providers who participate in DCI’s ACO and its affiliated ACOs (e.g., OneCare, LLC, etc.) (individually, an “
ACO Network Provider,” and collectively, the “
ACO Network Providers”); (b) enhance population based activities to improve the health of patients, including DCI’s aligned MSSP Beneficiaries; and (c) develop a platform designed to enhance the quality and efficiency of the services provided by the ACO Network Providers and improve health care in the communities served by DCI’s ACO, its affiliated ACOs, and the ACO Network Providers, all consistent with purposes of DCI and in furtherance of the goals of the MSSP ACO Model.
In connection with the Digital Care Integration Program, Deaconess will include participating ACO Network Providers in its website to allow for open access, transactional scheduling through the Epic electronic health record system. The initial ACO Network Providers that are anticipated to participate in the Digital Care Integration Program include Ferrell Hospital, Gibson General Hospital, Methodist Hospital, Methodist Hospital of Union County, Memorial Hospital and Health Care Center, and Good Samaritan Hospital (the “
Participating Hospitals”); Orthopaedic Associates, Inc., Evansville Surgical Associates, Inc., Women's Health Care, P.C., Neurosurgical Consultants, LLP, independent OneCare providers and physicians employed by Participating Hospitals. Based on the success of the program in achieving the Program Goals, DCI may expand the program to time to time to include additional ACO Network Providers.
