A message from Dr. Allen White, Vice President of Deaconess Clinic & Ambulatory Practice Operations

Deaconess Clinic has over 160 physicians and advance level practitioners in 30 locations providing a wide variety of health services. We have a large primary care group that partners with 19 different specialties to serve the community, along with a variety of procedural and hospital services.  We also provide Urgent Care services at two locations to meet patients’ needs after-hours, on weekends and holidays.
Deaconess Clinic was formed in October 2008 from the merger of the former Welborn Clinic and Deaconess Medical Group.  Since then we have been growing rapidly.  We continue to seek to fill the needs of our patients and our communities by adding essential physicians  in  a variety of specialties and locations.
We have the advantage of being part of Deaconess Health System.  The Clinic together with DHS has been at the forefront of technology and changing practice workflows.  This is demonstrated by 19 practice sites and 65 Internal Medicine and Family Medicine physicians achieving 2014 Level 2 recognition by the National Committee for Quality Assurance Patient-Centered Medical Home Program for 1/13/2015 - 1/13/2018.  This allows us to focus on the whole patient using a team based approach.  We want to help physicians focus their time on their patients using the skills they spent years learning, not filling out forms, looking up data, or completing records.
We use Epic, an Electronic Medical Record that allows access to your records by any physician in the DHS  that is caring for you.  Your test results, x-rays, medical history and other information your doctor needs to make good decisions is right at their fingertips.   This improves safety, improves overall efficiency and reduces risk of repeating tests unnecessarily.
We strive to improve the ways we communicate so we can get information to the patient faster and with fewer errors.  Our patient portal, MyChart is a great tool for this.  It allows you to review your diagnoses, test and x-ray results, immunization history, allergies, medications, and allows you to communicate requests for prescription refills, appointments, and minor medical questions directly to your physician.
We have many other projects upcoming including upgrading our Electronic Medical Record to have even more functionality. A few of the other things we are working on are innovative ways for you to be able to access health care more easily, improving transitions of care when leaving the hospital to reduce readmissions and improve outcomes, and working to provide more comprehensive services to nursing home patients in the region.
