Technology innovations to keep people living safely and independently are continually being developed and introduced. Deaconess VNA Plus offers the following in-home options:
CareLink Medical Alert Service
CareLink provides a 24-hour connection to professional responders at the push of a button. If you fall, have a medical emergency or need any type of assistance, you simply press your waterproof pendant. CareLink Support Staff will access your personal profile, determine your needs, contact a neighbor, loved one or emergency services depending on your situation, and stay on the line with you until help arrives.
CareLink is compatible with landlines, cellular service or homes with no phone line; and additional enhanced options include fall detection service and 24-hour access to a registered nurse. There is no equipment to buy and no long-term contracts to sign; and Deaconess VNA Plus staff will personally install the unit, test it and be available for ongoing questions and concerns.
CareLink service is currently available in Dubois, Gibson, Perry, Posey, Spencer, Vanderburgh and Warrick Counties in Indiana; Henderson County in Kentucky; and Edwards, Gallatin, Hamilton, Saline, Wabash, Wayne, White and Williamson Counties in Illinois.
CareMed Medication Management Service
Complex medication schedules can lead to mistakes: missing doses, taking incorrect amounts or taking pills at the wrong time. The CareMed MedMinder features compartments with pre-filled medications in removable cups; the compartment flashes when it's time to take the medication and also provides an audible message reminder if the cup is not removed within a specific timeframe (optional automatic phone calls, text messages and emails are also available.) MedMinder keeps details records of the medication activity, and caregivers may access the information via the Internet or elect to receive real-time email or text message notifications; weekly reports are also available. A Deaconess VNA Plus registered nurse can teach the caregiver how to fill the medication organizer; or for an extra fee, can fill it for the patient on a weekly or bi-weekly basis.
CareMed service is currently available in Gibson, Posey, Vanderburgh and Warrick Counties in Indiana.
For patients with specific chronic health conditions, Telehealth is a daily monitoring program that helps your home health nurse remotely track your vital signs and symptoms. Small devices can be set up in your home to measure your weight, blood pressure, heart rate, glucose level and blood oxygen level and transmit this data wirelessly to your nurse; the nurse can then view a status report, note if there are any significant changes and determine if adjustments to your care need to be made and your physician contacted. This process allows clinicians to detect even the smallest change in your status, which helps break the cycle of emergency care visits and hospitalizations. There is no equipment to buy; Telehealth results are sent automatically through your telephone line (toll-free), and there are no interruptions to your phone service.
