The Family Medicine Center is staffed with physicians from leading universities and medical centers, all across the nation. These dedicated physicians have chosen to specialize in Family Medicine at Deaconess Hospital. Consultation is provided by on-site board certified physicians with more than 50 years of combined experience.

Offering:

• Comprehensive health maintenance and medical care to the entire family regardless of age or type of problem
• Free immunizations for infants and children
• Maternity care and delivery
• Work and school physicals
• Care for occupational and sports injuries, including minor surgeries and repair of lacerations
• Flu shots, pneumonia and hepatitis B vaccines and free adult tetanus boosters

Appointments:

Office visits are by appointment only and may be scheduled by calling (812) 450-3363 from 8 a.m. to 4:30 p.m. Monday through Friday. If you need to change or cancel an appointment, please call 24 hours in advance.

Emergencies:

A doctor is available 24 hours a day at
(812) 450-3363 for emergencies. If possible, please call your doctor before going to the hospital Emergency Room. If you cannot reach the doctor, go directly to the Deaconess Hospital Emergency Room.

Prescriptions:

Medications are not routinely prescribed over the telephone except in certain post operative conditions, or chronic or terminal illnesses. However, we will be happy to provide telephone refill orders to your pharmacist for long-term medications if you are continuing under our active care.

Payment and Insurance:

We request that payment be made at the time of service. As a service to you, our staff will file your insurance claims if you are covered by an insurance plan. Your co-payment is due at the time of service. Please remember to bring your insurance card with you.

Your health insurance policy is an agreement between your insurance company and you, and there are many different policies. It is, therefore, your responsibility to know which services your plan covers. If you are uncertain, please contact your insurance company.
