MyChart is a secure, online health management tool that connects MyChart patients to their personalized health information - from anywhere, any time of day.  MyChart allows you to view your outpatient information such as test results, renew prescriptions, schedule appointments, and even send messages to your doctor.*

Remember MyChart is not to be used for emergency or urgent needs.  For medical emergencies, dial 911.

How Do I Get A MyChart Account?

MyChart is available at Deaconess Hospital Main, Deaconess Gateway, Deaconess Riley Hospital for Children, The Heart Hospital, Cross Point, all Deaconess Clinic locations,  Deaconess Critical Care, Deaconess Family Practice and Residency, and Deaconess Primary Care for Seniors physician offices.

Contact your doctor’s office for information about signing up, sign up during your next office visit, or request an activation code online.

MyChart signup for patients under the age of 18, or for access to a patient's MyChart as a caregiver, is available only at the patient's doctor's office.

If you are unsure whether or not you have a MyChart doctor, search find a doctor and look for the MyChart emblem in your search results. 

MyChart contains inpatient and outpatient test results and information.

If you have technical questions, you can call 812/450-3742 from 8:00 am to 5:00 pm Monday through Friday to speak with a customer service representative.
