Qualifications for Home Health Care
 
  • Have a skilled nursing or therapy need
  • The person must not leave home frequently (Only going out for medical appointments, worship, or special-occasion activities)
  • Have a physician’s order stating the need for nursing or therapy services
Because home health is not intended to go on indefinitely and is provided on an intermittent (part time) basis, an important aspect of this care is teaching the patient and family (or other caregivers) how to care for themselves. Deaconess Home Health's plan of care involves a team collaboration from all staff members to provide a holistic approach to patients' needs.

Deaconess Home Health works with the Accreditation Commission for Health Care (ACHC), an independent, private, not-for-profit organization, for quality assurance and accreditation. ACHC was established by home care and health care industry providers with the goal to provide an accreditation program that helps organizations improve operations and the quality of patient care and services they provide. ACHC has developed comprehensive standards and guidelines and partners with Deaconess Home Health to provide the tools and resources needed to deliver the best patient care.
