Self Management Training


Comprehensive, Individualized Training

Our comprehensive diabetes outpatient program empowers patients by helping them understand diabetes and how to control it. The program involves 10 hours of Diabetes Self-Management Training (DSMT) with an additional hour of follow-up during the initial 12 months. Physician referral is required.

Diabetes Self-Management Training
Our intensive DSMT program is taught in a group setting but is adapted to meet the needs of individual participants. Topics include:

  • Clinical Assessment/Behavior Change Education
    This is the first step in DSMT. During clinical assessment Deaconess Diabetes Center staff members record height, weight, blood pressure and a recent hemoglobin A1C level. Behavior change education addresses the typical stages of behavior change, obstacles to change, overcoming barriers, choosing behaviors to change, goal setting and problem solving.
  • Basic Diabetes Management
    The program addresses types of diabetes, factors affecting blood glucoses, blood glucose monitoring, insulin self-management, options for insulin delivery, oral medication self-management, acute and chronic complications of diabetes, foot care, the basics of exercise, stress management, and diabetes during pregnancy.
  • Meal Planning
    Topics include learning to use an individualized meal plan, carbohydrate counting, alcohol and diabetes, measuring food portions, eating out, reading food labels, and understanding the difference between carbohydrate, protein and fat.

Follow-up sessions
To ensure the continued success of our patients, we schedule one-hour follow-up sessions during the first 12 months of the initial training program. We will recheck your A1C, weight and blood pressure, and give you concrete information about the changes you have accomplished. We will review your initial self care behavior-change goals and discuss maintenance of diabetes health.

Physician Referral
A physician referral is required for participation in this outpatient program. Most insurance companies will require a physician referral before paying for our services, so this is a benefit to you. When  we receive a referral form from your physician, we will call you to schedule an appointment.
