Patient Pre-Registration

We offer online and phone pre-registration to help you make your upcoming visit more pleasant by registering before you arrive.

We understand that your time is valuable. To reduce your time in the registration process at the hospital, please pre-register using the online form.

For Deaconess Hospital, Deaconess Gateway Hospital and the Heart Hospital

For The Women's Hospital

• If you are wanting to pre-register at The Women's Hospital, please use The Women's Hospital's pre-registration form >>
• You may pre-register online at any time of the day. You are welcome to call the Patient Registration department or a Pre-Registration associate to answer any questions.
• If your information is incomplete or needs clarification, the staff may need to complete the registration by telephone or when you arrive.
• You will need to complete the registration process at the Patient Registration area upon your arrival at the hospital.

Most procedures require that you pre-register online or by phone.  If you have questions about registration please contact Patient Registration at 812/0450-2408. Patient Registration is located at  Deaconess Hospital and the Gateway Hospital campus. Patient Registration can easily be found near the main entrance at both campuses.
