Going Home

Your Discharge

When your doctor decides you are ready to leave the hospital, a discharge order will be written. You may want to make arrangements with a family member or friend to help you when it's time for you to go home. Your doctor or nurse may give you instructions to follow after you leave the hospital. These instructions may include restrictions on diet, activity or other matters. If you have any questions about these instructions, please be sure to ask at this time.

Personal Belongings

Collect all of your belongings and double-check closets and drawers. If you have any valuables stored in the hospital safe, please let a staff member know. Our staff will have your items returned to you before you leave.

Post-Hospital Care

Some patients require medical care or other assistance after hospitalization. Examples include skilled nursing care, home health care, home medical equipment, transportation assistance and more. To assist you as you plan for these needs, we have included a section in our Web site for Post-Hospital Care. For additional information, please call the Case Management Department at 812/450-3455. Our specialized social workers and nurses will be happy to assist.
