Deaconess Care Integration, LLC Public Reporting Information

A Next Generation Accountable Care Organization (ACO) is an ACO Model implemented by CMS. ACOs are groups of doctors, hospitals, and other health care providers that come together to provide coordinated, high quality care to their Medicare Fee-for-Service patients. Coordinated care helps ensure patients, especially the chronically ill or those who are hospitalized, get the right care at the right time, and avoid unneeded duplication of services such as lab tests and x-rays. The Next Generation ACO Model offers different methodologies and financial arrangements with higher levels of risk and reward than current Medicare ACO initiatives.

Joint ventures between or among the Next Generation Participants and Preferred Providers

This is a joint-physician owned facility between Deaconess Health System and area cardiologists and cardiovascular surgeons of Heart Physicians, LLC.
 
Neurosurgery:  Mainspring Managers 
In an effort to improve coordination of care and create programs that will result in achieving the “Three Part Aim” of providing improved care for the Medicare Fee-For-Service beneficiaries we serve, better health for the populations we care for, and doing so in a financially sustainable manner, Deaconess Hospital and Neurosurgical Consultants, LLP have formed Mainspring Managers.

Orthopedics:  Ortho Align
In an effort to improve coordination of care and create programs that will result in achieving the “Three Part Aim” of providing improved care for the Medicare Fee-For-Service beneficiaries we serve, better health for the populations we care for, and doing so in a financially sustainable manner, Deaconess Hospital, Deaconess Care Integration, and participating Orthopaedic Associates physicians have partnered to create Ortho Align Joint Venture. Deaconess Care Integration – Ortho Align Joint Venture which will include all orthopedic surgeries at the Gateway campus. 

Vascular Surgery:  Vascmed, LLC
In an effort to improve coordination of care and create programs that will result in achieving the “Three Part Aim” of providing improved care for the Medicare Fee-For-Service beneficiaries we serve, better health for the populations we care for, and doing so in a financially sustainable manner, Deaconess Hospital and Evansville Surgical Associates, LLC have formed Vascmed, LLC.

Shared Savings & Losses

  1. The amount of any Shared Savings or Shared Losses for any Performance Year
    1. Medicare Shared Savings Program Performance Year 3:  $0
    2. Next Generation ACO Performance Year 1:  Results will be made available later in 2017
  2. The proportion of Shared Savings invested in infrastructure, redesigned care processes, and other resources necessary to improve outcomes and reduce Medicare costs for Beneficiaries
    1. The proportion of shared savings to be invested in infrastructure depends on the amount of shared savings received. Currently, the distribution policy covers operating expenses for the ACO with distribution across membership for the remaining funds.
  3. The proportion of Shared Savings distributed to Next Generation Participants and Preferred Providers
    1. The current Next Generations Participants receive incentives for completing work designed to achieve improvement in patient experience, quality performance, and total cost of care reduction.  These incentives are paid regardless of the shared savings results.
